Enfield Disability Action-020 8373 6228 - Calls are answered between 10am and 4pm Monday to Thursday. They can give advice over the phone and can meet with a parent to complete the form.

Enfield Carers Centre – 020 8366 3677 can also provide advice around DLA. Centre Opening Times: Monday to Friday 09.00-17.00

Drop In: Monday-Friday 10.00-12.00 & 14.00-16.00 

School can support with the section on the form about the child and this can be completed by the child’s Phase Manager.
